The Ontario Volleyball Association Insurance Program is available to fully registered members of the OVA.  The OVA Insurance Program outlines general liability and sport accident coverage.  To find out about details about coverage please view the Ontario Volleyball Insurance Program document listed below.

Certificate of Insurance

In order for a Club or Recreational League registered with OVA to obtain a certificate of insurance, the Club or Registered League main contact must fill out the Insurance Request Form (word version) (PDF version). 2012-2013 insurance requests will be valid up until August 31, 2013. The following information is required to be filled out on the Insurance Request Form (word version) (PDF version):

1.  Name of the school board(s) & school(s). (please use full school board names, no acronyms)
2.  Name of the Club contact person.
3.  Email address for the Club contact person.
4.  Name of the OVA Club.

Please allow fourteen (14) business days for your request to be processed once it has been received by the OVA office.
